Patient Advocate Foundations Scholarships (Undergraduate/Scholarship)

Purpose: To provide financial support to individuals under the age of 25 who have been diagnosed with cancer or another life-threatening disease. Focus: General studies. Qualif.: Applicants must be survivors or current patients diagnosed with cancer or another life-threatening disease; must write an essay on how their diagnosis has impacted their lives and their future goals, (1000 word maximum); must submit a copy of an acceptance letter from their chosen college, university, or vocational-technical school; must submit written documentation from physician of medical history; must provide two letters of recommendation from non-related persons (examples: teachers, coaches, community leaders). Please ask that each reference include their name, address and phone number within the letter; must provide an official high school and/or current college transcript (with school seal); financial form that demonstrates financial need with a copy of the first two pages of tax returns. Criteria: Preference will be given to those who meet the criteria.

Funds Avail.: $3,000. To Apply: Applicants must maintain an overall 3.0 G.P.A.; must be full-time students; must sign an agreement to complete 20 hours of community service for the year the scholarship will be dispensed and provide PAF with a written confirmation from a supervisor when that service has been completed.
